MRI Imaging with Magnetic Contrast
MRI tunes magnetic pulses to weigh water differently, so soft tissues separate without any radiation.
Why Does This Exist?
CT sees calcium and blood well but flattens soft-tissue shades. Brains and joints need water contrast. MRI excites hydrogen spins and listens as they relax with T1 and T2 times per tissue. Input is a pulse sequence choice; output is weighted volumes where cerebrospinal fluid, white matter, and tumor order differently per sequence. No ionizing dose accrues.
Think of It Like This
Tuning guitar strings to hear wood types
Plucked strings of one guitar fade at different rates by wood and varnish. Listening early hears attack, listening late hears sustain, and each timing favors some woods. No single timing shows every wood best.
Echo times are listening moments and sequences are pluck styles. The analogy stops at safety: strong magnets pull metal, not just sound.
How It Actually Works
T1 weighting uses short TR and TE so fat is bright and fluid dark. T2 weighting uses long TR and TE so fluid is bright. FLAIR nulls fluid to show edema beside ventricles. Gadolinium shortens T1 where vessels leak, lighting active tumor rims. Motion during the minutes-long scan rings across phase-encode direction.
Worked example
A cm metastasis reads isointense on T1, bright on T2, and rim-bright post-gadolinium with core dark. Edema around it spreads cm on FLAIR but stays invisible on CT windows. Measuring diameters on T1-post plus FLAIR separates active tumor cm from edema cm total, which changes the tumor segmentation target.
Watch Out For
Motion ringing mistaken for syrinx
Swallowing during cervical scans paints repeating ghosts down the cord. The symptom is a false central canal line. Check phase direction ghosts outside anatomy and rescan with gating.
The Quick Version
- MRI weighs T1 against T2 to split soft tissues without radiation.
- Sequence choice decides which tissue pops; one sequence never suffices.
- Gadolinium lights leaking vessels at active tumor rims.
- Do not measure edema on T1 alone; add FLAIR for fluid contrast.
- Motion rings along phase direction and mimics thin lesions.
